If you are a NEW poll worker, you will need to submit the three personnel forms listed below to the Board of Elections.   Please print, complete and return the required forms to the Board of Elections by either emailing them to This email address is being protected from spambots. You need JavaScript enabled to view it., or by faxing them to (845) 808-1920.  

If you do not submit the required forms prior to your scheduled class, you should plan to arrive at least 10 minutes earlier to complete them at that time.
