New Poll Worker Required Personnel Forms

If you are a NEW poll worker, you will need to submit personnel forms to the Board of Elections.  Please print, complete and return them to the Board of Elections by emailing them to This email address is being protected from spambots. You need JavaScript enabled to view it., faxing it to (845) 808-1920.

If you do not submit the forms prior to your scheduled class, you should plan to arrive at least 10 minutes earlier to complete them at that time.
