EISEP COORDINATOR
DISTINGUISHING FEATURES OF THE CLASS: This is professional and administrative work involving the coordination, administration, and daily operation of the Expanded In-Home Services for the Elderly Program (EISEP). An incumbent in this position is responsible for planning, implementing, coordinating, and evaluating program operations to ensure compliance with New York State Office for the Aging (NYSOFA), county, and funding requirements. Responsibilities include supervision of professional and clerical staff; fiscal management; contract monitoring; client service authorization; and program quality assurance. The incumbent exercises independent judgment in interpreting program regulations, resolving operational issues, and ensuring the efficient delivery of services that enable older adults to remain safely in their homes. General supervision is received from the Director and/or Deputy Director of the Office for Senior Resources. Performs related work as required.
TYPICAL WORK ACTIVITIES: (Illustrative only)
- Coordinates and administers all aspects of the Expanded In-Home Services for the Elderly Program (EISEP);
- Plans, assigns, and oversees the work of Home Health Aides (HHA), substitute typists, and other assigned staff;
- Coordinates Home Health Aide schedules, workloads, and client assignments to ensure efficient service delivery;
- Works collaboratively with contracted Home Health Agency providers to coordinate the staffing of Home Health Aides needed to meet program demands and communicates any performance concerns or service and staffing needs with agency supervisors.
- Interprets and implements NYSOFA regulations and program requirements;
- Exercises independent judgment in determining client eligibility, assessing service needs, developing care plans, and authorizing appropriate non-medical services;
- Conducts assessments, reassessments and develops care plans as part of client case file;
- Coordinates service delivery among clients, caregivers, and community agencies;
- Reviews, approves, and monitors provider invoices, aide documentation and timesheets, and program expenditures to ensure fiscal accountability and compliance with funding requirements;
- Maintains financial records and monitors program expenditures to ensure services remain within available appropriations;
- Monitors provider contracts for compliance with contractual obligations, state regulations, and program standards;
- Investigates provider deficiencies and coordinates corrective action when necessary;
- Maintains required program and client case documentation and prepares reports required by county, state, and federal funding agencies;
- Coordinates transportation needs related to program operations and service delivery;
- Maintains and manages program waitlists and allocates available program resources based upon client need and funding availability;
- Develops and maintains collaborative relationships with community agencies, healthcare providers, and other organizations serving older adults;
- Provides information and assistance to older adults and their caregivers regarding available community resources and benefits;
- Assists in evaluating program policies and procedures to improve service quality and operational efficiency;
- Performs a variety of related activities as required.

FULL PERFORMANCE KNOWLEDGES, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS: Good knowledge of the principles and practices of program administration and supervision; good knowledge of federal, state, and local programs serving older adults; good knowledge of community agencies, facilities, and services available to older adults; good knowledge of fiscal recordkeeping, contract monitoring, and program accountability; ability to supervise and evaluate the work of others; ability to interpret laws, regulations, policies, and procedures; ability to analyze administrative and operational problems and develop appropriate solutions; ability to establish and maintain effective working relationships with staff, contracted providers, clients, and community organizations; ability to communicate effectively both orally and in writing; ability to organize and coordinate multiple activities simultaneously; sound professional judgment; tact; courtesy; integrity.
MINIMUM QUALIFICATIONS*: Either
- Bachelor's degree in Social Work, Human Services, Public Administration, Gerontology, Nursing, or a closely related field and two (2) years of experience in aging services, human services, social work, case management, program administration, or a closely related field, one (1) year of which shall have included supervisory responsibilities; or
- Associate's degree in Social Work, Human Services, Public Administration, Gerontology, Nursing, or a closely related field and four (4) years of experience in aging services, human services, social work, case management, program administration, or a closely related field, one (1) year of which shall have included supervisory responsibilities.
PLEASE NOTE: Your degree must have been awarded by a college or university accredited by a regional, national, or specialized agency recognized as an accrediting agency by the U.S. Department of Education/U.S. Secretary of Education.
