Public Health Nurse
The Putnam County Department of Health is seeking qualified candidates for the position of Public Health Nurse.
This is a Competitive position under New York State Civil Service Law. Applicants must apply for and participate in an review of training, experience and licensure HERE.
DESCRIPTION OF DUTIES: This is professional nursing work with a focus on population health involving education, disease prevention, and assessment of health needs. Responsibilities include continuous surveillance and assessment of the community with primary goals of promoting health and wellness, preventing disease, disability, and premature death, while improving the overall quality of life for individuals, families, and community groups. Work may be performed at patients’ homes or in clinics or community settings. Special work duty outside of normal work hours (such as on holidays or weekends) may be required. Work is performed under direct supervision of the Supervising Public Health Nurse and general supervision of higher-level Health Department staff. Performs related work as required.
TYPICAL WORK ACTIVITIES:
- Conducts physical assessment by careful interviewing and use of nursing skills;
- Performs skilled nursing care and provides education and prescribed treatments to patients/clients , families, communities and populations in their homes, in clinic settings, in the community, and various off-site locations;
- Coordinates plans for service with Registered Professional Nurses, nutritionists, social workers, physicians and other professional workers acording to the client/family needs;
- Conducts disease investigation, contact tracing, evaluates completion of appropriate treatment, and provides education to prevent disease spread;;
- Participates in public health emergency preparedness activities;
- Educates, supports and empowers patients, families, communities and populations on strategies that create healthy environmental, social and economic conditions in which populations may thrive;
- Participates in public health accreditation activities which includes serving on committees, preparing and reviewing documents, and other activities as required
- Participates in the conduct of surveys and studies related to health matters;
- Documents services provided through records and referral forms, etc.;
- Provides ongoing needs assessment and care review for patients and monitors appropriateness and quality of services being provided.
- Collects, interprets, recommends and reports targeted surveillance data related to population health for application in assigned programs ( ex. CD, STDs, TB, Immunization); May teach classes, addresses groups, and participates in community planning related to nursing and health;
- May work with the Early Intervention (EI) Program to conduct Child Find home visits for the purpose of assessing and monitoring child development for those at risk
- May provide maternal and child health guidance;
- May provide Public Health Nurse services at migrant camps, and related referrals to the appropriate agencies;
- Performs a variety of related activities as required.
MINIMUM QUALIFICATIONS:
Bachelor’s degree in nursing and possession of a valid license and current registration to practice as a Registered Professional Nurse in New York State are required at time of appointment and must be maintained throughout employment
PLEASE NOTE: Your degree must have been awarded by a college or university accredited by a regional, national, or specialized agency recognized as an accrediting agency by the U.S. Department of Education/U.S. Secretary of Education.
SPECIAL REQUIREMENT:
Possession of a valid, unrestricted, appropriate level driver’s license, and acceptable driving record are required at time of appointment and must be maintained throughout employment.

SCHEDULE: Full-Time, Monday-Friday
STARTING SALARY: $80,116
