WHAT TO EXPECT

  • Please select the appointment time you are requesting.

  • This is an influenza clinic for individuals 18 years of age and older.
  • Residents should download and complete the form prior to your appointment.  Forms will not be available at the clinic.
  • The fee for vaccination is $30.00 for those 18 through 64 years of age.  Only cash or checks payable to Putnam County Department of Health will be accepted.
  • Those 65 and older or anyone with a Medicare card can receive the vaccine free of charge if they show their Medicare card.
  • High dose flu vaccine is being offered for seniors age 65 and older.
  • Short sleeves are required to make it easier for the nurses to administer the vaccine in a timely manner.
