The Putnam County Office for Senior Resources is seeking qualified candidates for the position of Registered Professional Nurse.

DESCRIPTION OF DUTIES: 

This is professional nursing work involving outreach work and initiation of health and wellness promotion and disease prevention programs. Responsibilities include assessment and periodic reevaluation of the needs and wellness of individual senior clients, program participants and/or family members. Provides skilled nursing care on site as needed Work may be performed at the Office for Senior Resources and/or nutrition sites and is performed under general direction. Performs related work as required.

TYPICAL WORK ACTIVITIES:

  • Assesses clients’ conditions, including client’s environment, economic factors, health history, and overall wellness;
  • Initiates health promotion and disease prevention program at nutrition sites and other designated areas;
  • Provides ongoing outreach to program participants regarding general health programs;
  • Counsels, guides and teaches individuals and families to recognize and address physical, emotional, spiritual and environmental health needs;
  • Works directly with clients or their designees to identify agencies and individuals potentially useful to the client;
  • Participates in and arranges for various activities and screening for health conditions, such as glaucoma, hypertension, cancer prevention services and hearing and vision testing using community resources;
  • Re-evaluates, at periodic intervals, the client’s progress, needs and goals;
  • Schedules various health programs and activities and coordinates on-going wellness clinics;
  • Disseminates information concerning service programs for seniors including preparing pamphlets, flyers, news releases, surveys, articles for newsletters involving wellness, outreach, health matters and related topics;
  • Coordinates plans for service with nutritionists, caseworkers, and other professional workers concerned with individual and family health care;
  • Documents services provided through records and/or referrals;
  • Provides skilled nursing care to clients at the Office for Senior Resource’s program and nutritional sites, as needed;
  • Performs a variety of related activities as required.

MINIMUM QUALIFICATIONS:

Completion of an accredited course of study for registered nursing.

SPECIAL REQUIREMENTS:

1.      Possession of a license to practice as a registered professional nurse in New York State at time of appointment and throughout employment.

2.      Possession and maintenance of a valid, unrestricted driver’s license, and an acceptable driving record, at the time of appointment and throughout employment.

PLEASE NOTE: Your degree must have been awarded by a college or university accredited by a regional, national, or specialized agency recognized as an accrediting agency by the U.S. Department of Education/U.S. Secretary of Education.

NOTE: Qualifications and requirements are in accordance with New York State Education Law, Article 139, Sections 6900 to 6910 (“Nurse Practice Act”)

SCHEDULE: 5-hour shift, Monday-Friday, 25 hrs/wk

STARTING SALARY: $50,660 per year
